Sadaqa Ward, LCSW — Executive Director (Founder)
Sadaqa Ward is a Licensed Clinical Social Worker and healthcare leader with extensive experience in clinical supervision, program development, and care coordination. As Executive Director of the GA Bello Foundation for Wellness, she leads the Foundation’s day-to-day operations and provides oversight of services that expand equitable access to whole-person wellness supports across Georgia, including short-term therapy access, care coordination, and community initiatives.
Azeez Bello — Director
Azeez Bello is a founding member of the GA Bello Foundation for Wellness and serves as a voting member of the Board of Directors. As a social work student, he brings an emerging perspective on community wellness, access to care, and youth and family supports. Azeez is committed to advancing the Foundation’s mission through outreach, program support, and community engagement across Georgia.
Alta Smalls — Secretary
Alta Smalls serves as Secretary of the GA Bello Foundation for Wellness and supports the Board’s governance and organizational accountability. In this role, she helps maintain clear records, meeting documentation, and follow-through on key decisions to keep the Foundation’s work moving forward with structure and transparency. Alta is committed to advancing whole-person wellness across Georgia and strengthening the systems that help community-centered programs operate effectively.
Toni Rose — Director
Toni Rose serves as a Director of the GA Bello Foundation for Wellness. She holds a Bachelor’s degree in Public Health and brings over 40 years of experience in healthcare services, including both direct care and administrative leadership. Toni’s work has long focused on advancing health equity through a practical understanding of health disparities and the social determinants of health. She supports the Foundation’s mission by strengthening program planning and community initiatives that improve access, stability, and whole-person wellness for individuals and families across Georgia.
Tracy Scoggins — Treasurer
Tracy Scoggins serves as Treasurer and provides financial oversight to support responsible growth and transparency. In this role, she helps guide budgeting, expense tracking, and financial reporting to ensure the Foundation’s resources are managed with care and aligned with mission priorities. Tracy is committed to strengthening the operational foundation that allows community-centered wellness programs to expand and thrive across Georgia.
Raheem Bello — Junior Advisory Board Member
Raheem Bello is a founding member and serves on the Junior Advisory Board, offering a youth perspective as the Foundation develops programs that support students, young adults, and families across Georgia. In this role, he provides input, feedback, and ideas to help ensure initiatives remain relevant, community-centered, and responsive to the needs of young people. Raheem also supports outreach and program efforts as needed, helping advance the Foundation’s mission to expand whole-person wellness supports throughout Georgia.
